Hey Mike; ineteresting delema - I can sure appreciate the desire for maximizing child safety though.
I merely wanted to note that I believe the primary reason for relegating children to rear seats is where air bags are concerned. An example would be the modern C5 vettes in which you can request a passenger side airbag defeat switch if you have children. Another is with pickups - standard and extended cab - not crew cabs - have air bag defeat switched for child seats. Doesn't necessarily help your situation - but I felt I should throw that in based on the way you mentioned the child seat up front.
As far as the addition of the shoulder belt. You may want to take a look at the volvos use of the "sleeper safety net" (not sure of the correct term). IT has attachement points that include an area that may be adaptable to your shoulder belt.
